What does your dizziness feel like?

People describe dizziness in very different ways. What brings your symptoms on can provide important information during your assessment.

The room spins when I roll over

Brief spinning when lying down, getting up, looking upward or turning in bed can occur with positional vertigo.

I feel off-balance or like I'm on a boat

Some vestibular problems feel more like swaying or instability than true spinning.

Busy places make me feel dizzy

Grocery aisles, crowds, patterns or moving environments can sometimes trigger visual motion sensitivity.

Moving my head makes things feel unstable

Head movement may cause dizziness, imbalance or difficulty keeping vision clear and steady.

What is vestibular physiotherapy?

Your balance depends on information from your inner ears, eyes, brain, muscles and joints working together. When one part of this system is not working normally, you may experience dizziness, vertigo, imbalance or difficulty with movement.

Vestibular physiotherapy uses assessment and individualized rehabilitation to understand how these systems are working together. Treatment is based on your symptoms and findings — not every person with dizziness needs the same exercises.

Does the room spin when you roll over in bed?

One possible cause is benign paroxysmal positional vertigo (BPPV) . BPPV can cause short episodes of spinning when you change the position of your head, such as rolling over, lying back, looking upward or bending down.

Positional testing can help determine whether symptoms follow a pattern consistent with BPPV. When BPPV is identified, the affected ear and inner-ear canal can influence which treatment or repositioning technique is appropriate.

Tried the Epley maneuver but you're still dizzy? Not every case of positional dizziness is the same. Persistent symptoms may need a more complete assessment rather than repeatedly performing the same maneuver.

  • Visual motion sensitivity
  • Neck movement when relevant

Frequently Asked Questions

Why does the room spin when I roll over in bed?

Brief spinning triggered by rolling over, lying down, looking up or changing head position can occur with positional vertigo such as BPPV. An assessment can help determine whether your symptoms fit this pattern.

Can physiotherapy help with BPPV?

Physiotherapists with vestibular training can assess positional vertigo. When BPPV is identified, canalith repositioning techniques may be appropriate depending on the affected ear and canal.

Why didn't the Epley maneuver work for me?

Not every case of positional dizziness is the same. The affected ear, inner-ear canal and underlying cause can influence which treatment is appropriate. Persistent symptoms should be properly assessed.

Why do grocery stores or busy places make me dizzy?

Busy visual environments can challenge the way the eyes, brain and vestibular system process movement and orientation. Some people experience increased dizziness in grocery stores, malls, crowds or other visually complex environments.

Can vestibular exercises make me dizzy at first?

Some exercises may temporarily bring on mild symptoms because they gradually challenge the balance system. The goal is not to overwhelm you. Exercises should be selected and progressed according to your response.

Can vestibular physiotherapy help after an inner-ear problem?

Vestibular rehabilitation may help with persistent dizziness, imbalance or motion sensitivity following some inner-ear conditions. Treatment depends on the cause and findings from your assessment.

Can it help with dizziness after concussion?

Dizziness and balance problems can occur after concussion. Vestibular rehabilitation may be part of concussion rehabilitation when assessment identifies vestibular or balance-related problems. You can also read our concussion physiotherapy page .

How long does vestibular rehabilitation take?

It varies. Treatment length depends on the cause of dizziness, how long symptoms have been present and how you respond to rehabilitation. Your physiotherapist can provide a more individualized plan after assessment.
